Clubs who have outstanding fines must have these fines paid by 31-5-2012 if not paid admittance to the AGM in June will not be allowed.

Once you download it fill it in, then get it signed by all club officials as appropriate then post it to Andy Sandilands along with a cheque for the league fees and western district and good faith bond (new teams only) please note that if your application form is in before the 20th July then the League fee for the season is £60.00 after that it is up to £75.00 all leagues will be closed when the match secretary has filled the age group.
